Tutor Notes and Quarterly
Hours Form
Literacy of Northern New York is required to collect data from
tutors regarding their time spent working with students. We are
also looking for any information about changes in your students'
lives. If your student takes a course, gets a job, enters any
kind of training, has a reduction in Public Assistance, etc., let us know.
This helps us with our reports for funding. We are looking for goals
and goals achieved. Tell us about any time you spend with your
student such as an outing or working on a computer. If you are not sure,
just send the information and we will determine what is relevant. You
can send notes any time. Please
complete a separate form for each student you are working with after
every three month period when submitting hours. When you are finished with each form, click submit. You will see a
confirmation page. If you experience any problems, contact the
